Effective problem solving is critical for the success of any business, yet we often see companies spending large amounts of time, resources & money wrestling with consequences and not getting to or solving root cause/s. In order to do this effectively we need to overcome our human biases and prevent jumping to cause & solutions. These courses covers far more than just taking trainees through the theory and tool/s.

INCIDENT INVESTIGATION TRAINING:
(What happened? What were the holes in the layers of defence? What other holes are there in the layers of defence?) How can we prevent this incident happening that way or any other way?)
Based on James Reason's Swiss Cheese Model, this course takes trainees through the layers of defense to come up with plausible scenario's of how the incident happened and how else it could happen and allow us to address the holes in the layers of defense.
